Doctors Without Borders has worked in India since 1999, providing free-of-charge essential healthcare to people in remote areas; treatment and care for people affected by HIV/AIDS, malnutrition; hepatitis C, tuberculosis, kala azar and sexual and gender-based violence; an important subject for our action is containing effect of antibiotic resistance. We also respond to natural disasters and other emergencies, provides mental healthcare and advocates for the development of more effective and affordable medicines to improve access to treatment everywhere.

Doctors without Borders was awarded the Indira Gandhi Prize for Peace, Disarmament and Development in 1996 and the Nobel Peace Prize in 1999.

Doctors Without Borders India (DWBI) and the endTB project

Doctors Without Borders, and its partners i.e. Partners In Health (PIH), Interactive Research and Development (IRD), Epicentre, France, Institute of Tropical Medicine, Belgium and the Harvard Medical School have launched two major clinical trials, endTB and endTB-Q which seek to revolutionize treatment for the toughest strains of tuberculosis (TB), the world’s leading infectious disease killer. The goal of these studies is to generate high quality evidence from a highly heterogeneous population that could lead to a change in the global recommendations for the management of MDR-TB.

The endTB trials are multi-country, randomized, controlled, Phase III clinical trial testing five new, all oral, shorter duration regimens compared to the current standard of care. The aim of the trials is to find shorter, less toxic and injection-free treatments for ‘multidrug-resistant TB’ (MDR-TB). The experimental regimens mainly rely on newly approved or repurposed drugs and avoid drugs to which high rates of resistance have been reported in MDR-TB patient populations.

These multi-country trials are being conducted in Georgia, India, Kazakhstan, Lesotho, Pakistan, Peru, South Africa and Vietnam. In India the trials are conducted at Mumbai and Pune sites by Doctors Without Borders India (DWBI) in Partnership with ICMR- National AIDS Research Institute (NARI) and in support with National Tuberculosis Elimination Program (NTEP).

Main Tasks & Responsibilities

General Activities:

• Undergo training in the protocol, trial procedures, and GCP: Participate in the planning, implementing, monitoring and evaluation of medical activities for DRTB patients of endTB and endTB-Q trials. Provide clinic based nursing assessment planning and care for new and follow up of end TB trial patients and record accurately in patients file.
• Engage with clinic staff, and assist endTB trial staff and DWB counselors to identify and recruit potentially eligible rifampicin-resistant TB patients for the trial.
• Triage sick walk-in patients and prioritize them accordingly. Provide emergency nursing care as necessary in designated area informing the doctor promptly of patient condition.
• Check vital signs, weights and BMIs are being taken and recorded accurately in patient charts
• Assist Registrar to maintain correct patient flow in the clinical area and report any problems to the Nurse Supervisor.
• To carry out activities like injections, dressings, blood withdrawal, sputum collection, ECG in collaboration with other nursing staff as per DWBI SOP’s and protocol and follow all Infection control procedures.
• Call patients for a change in medicines investigations to be done, as per instructions given by Doctors.
• Distribute the study drugs to study participants as dispensed by the pharmacist on prescription of medical doctor.
• Attend calls from patients (including weekend)
• Work as back up to clinic team
• Liaise with PI, Medical Coordinator, Medical officer and Site coordinator for efficient medical management of trial participants.

Port a Cath/PICC line Care and Management / Infection Control / Laboratory Samples / Community/Field Work / Hospitalization / IEC and Health Promotion

• Conduct Needle Change/ Dressing for DRTB patients with Port-a-Cath/PICC line as per DWBI protocol. In case of any complication, inform Doctors and Nurse Supervisor immediately. For Complications with respect to Port/PICC, actions to be followed as directed by Doctors/ Nurse Supervisor.
• Be aware of and strictly follow all staff and patient infection controls instituted for the prevention of the spread of TB in the clinic. This includes ensuring all environmental and administrative controls are adhered to according to clinic infection control policy
• Report Bio-medical equipment malfunction to Nurse Supervisor immediately.
• Manage biomedical waste in endTB clinic
• To record patient details accurately in the Laboratory referral form and register and ensure that samples are given to endTB project identified labs at time of scheduled collection.
• To maintain cold chain and other logistic requirements of sample storage.
• Report any problems to the Nurse Supervisor.
• Provide health education, nursing care, adherence counseling, identify side effects for endTB trial subjects with patient support and medical staff.
• Foster good rapport with hospital staff – be responsible for clear and regular communication including sharing reports and results for hospitalized patients.
• With the support of the medical team, promote the plan of care for the patient.
• Give feedback daily to the clinic team and patient support team. If any issues inform to Nurse Supervisor and discuss in dashboard meeting.

• Languages: English, Hindi/Marathi.

Qualifications: BSc (Nursing) or General Nursing & Midwifery (GNM) recognized from a Maharashtra Nursing Council.

Experience:
• Desirable 2 years of previous experience of clinical trial
• Desirable experience of TB OPD clinic
