Doctors Without Borders/ Medecins Sans Frontieres (MSF) is an international, independent, medical humanitarian organisation that delivers emergency aid to people affected by armed conflict, epidemics, natural and man-made disasters and exclusion from healthcare in more than 70 countries. DWB offers assistance to people in need and irrespective of race, religion, gender or political affiliation. Doctors Without Borders has worked in India since 1999, providing free-of-charge essential healthcare to people in remote areas; treatment and care for people affected by HIV/AIDS, malnutrition; hepatitis C, tuberculosis, kala azar and sexual and gender-based violence; an important subject for our action is containing effect of antibiotic resistance. We also respond to natural disasters and other emergencies, provides mental healthcare and advocates for the development of more effective and affordable medicines to improve access to treatment everywhere.

Doctors without Borders was awarded the Indira Gandhi Prize for Peace, Disarmament and Development in 1996 and the Nobel Peace Prize in 1999. Doctor without Borders India (DWBI) runs projects in the states of, Bihar, Chhattisgarh, Delhi, Jammu and Kashmir, Jharkhand, Maharashtra, Manipur, and Telangana.

Doctors Without Borders India’s (DWBI) Drug-Resistant Tuberculosis (DRTB) project in MUMBAI;

DRTB freeTreatment, Care and Support

• The collaborative project within NTEP [National Tuberculosis Elimination Program] for DRTB – patient management at Shatabdi Hospital, Govandi – OPD, community outreach medical and patient support activities in M/East Ward area
• Medical, “Psychosocial Support” and “Infection Prevention and Control” in GTB Hospital, Sewri
• The independent clinic in Govandi to provide free and quality care to the most severe cases of DRTB

Task and Responsibilites

• Pediatrician is going to be part of the multidisciplinary team to provide medical care for DRTB pediatric and adolescent patients in all the sites of DWBI intervention, especial focus required in small children DRTB patients.
• Note: New-borns care is not part of the DRTB intervention in Mumbai project.
• Applying medical knowledge and skills to prevention, diagnosis and treatment of children, ensuring integrated new-born and paediatric care through Emergency, OPD, and IPD services in New-borns and cases of malnutrition. Prescribing the necessary treatment, respecting DWBI protocols.
• Collaborating closely with the different members of the team, Knowing and ensuring all updated DWBI medical protocols and universal precautions are followed and implemented, reducing bio-hazard risks and improving infection control.
• As an expert in paediatrics, proposing strategy (modification on current new-born and paediatric care management) and implementing them after validation of Operation and Paediatric Advisor.
• Ensuring on-going new-born and paediatric training (formal and bedside) of the medical/paramedical multidisciplinary team in order to optimize the new-born and paediatric quality of care. Reviewing and discussing selected complicated cases or mistakes and giving technical input.
• Keeping patients and their families informed of the illness and the treatment to follow, checking that they have been understood. Ensuring professional autonomy and confidentiality are respected and a good attitude and proper bedside manner towards the patient and their care takers and the rest of the medical team.
• As an expert in paediatrics, lead the and adjust the current medical strategy to further improve on preventive/diagnostic and treatment tools including contact tracing for better outcome among the paediatric DRTB cohort and implementing them in all the sites of intervention after validation of Operation and Paediatric Advisor. especial emphasis to improve the diagnostic of DRTB in small children.
• Provide medical consultation for paediatric patients, especial emphasis in small children.
• Be part of the DR TB nodal center Committees at MEW and MSF Clinic and ensure quality of clinical decision making, develop tools for documentation of DRTB pediatric patients to facilitate case discussion and decision of treatment regimen by the nodal center committee. Refer to Mission and SAMU Tech Ref for complicated questions.
• Develop decentralised OPD model of care for paediatric DRTB patients focus in the PHCs , Shatabdi hospital and MSF clinic. Define referral system IPD for critical paediatric patients.
• Develop training modules about DRTB care for paediatric and adolescents patients and make a training plan for DWBI medical team and external partners if needed.
• Assess and make a plan of integration of DRTB paediatric care into Shatabdi hospital and PHCs in MEW.
• Collaborating closely with various OPD/IPD and outreach facility to ensure MSF as well as program Guideline implementation as well as IC measures are followed and implemented .
• Visiting community/HP with outreach team for better understanding of community needs in paediatric DRTB cases as well as capacity building of the HCW at community /HP level.
• Ensuring effective contact tracing at community level as well as with linking to the two OPD for further management of cases.
• Visiting IPD facilities where our cases would be admitted, collaborate with the treating doctors for joint management of such cases as well as to get update on regular basis of our hospitalized patients. Lay down the admission, transfer and discharge criteria of such cases. Fill in the Pharmacovigilance forms in case of Adverse drug reactions.
• Provide inputs and contribute the revision and validation of the PS counselling tools for DRTB paediatric patients. along with MTR
• Sensitization and awareness sessions will be done for paediatricians and medical staff of Shatabdi hospital and PHCs.
• Develop undernutrition assessment among the DRTB paediatric patients together with the medical team and nutritionist. Contribute to implement the tools to identify undernutrition, treatment plan and follow up.
• Be a backup in case there is shortage of the national doctors for seeing adult cases.
• Be part of the AMR stewardship committee and assess quality of medical prescription every 6 months.
• Contribute and support the ORs related to DRTB paediatric.
• Represent MSF in general Paediatric discussion at local or national level.
• To be flexible to rotate all interventions of the project

Qualifications: Essential-

• MBBS degree + PG dipoloma or Degree in paediatric
• Valid registration from Medical Council of India or state medical councils

Experience:

• Pediatrician with 3 years of experience working as pediatrician in the hospital.
• Desirable working experience with DWBI or other NGOs
• Desirable working experience in developing countries

Duration – 1 Year (Renewable)
