BACKGROUND

Globally, Non -Communicable Diseases (NCDs) are the leading cause of mortality accounting for 70% of the total mortality or nearly 40 million deaths each year. Nearly, 80% of premature deaths (between 30-69 years of age) are caused by only four disease groups namely, cardiovascular diseases (~18 million deaths), cancers (~9 million deaths), respiratory diseases (4 million deaths) and diabetes (~2 million deaths). Nearly 75 percent of NCD deaths – and 82 percent of premature NCD deaths (i.e. those occurring before the age of 70) – occur in low- and middle-income countries (LMICs). In India, NCDs contribute to nearly 60% of all-cause mortality claiming nearly 5.87 million lives each year. The prevalence of obesity and overweight is also showing a rapid increase in the country. Age standardized prevalence of obesity (BMI= 30) has increased by 22 % in the span of four years (2010-2014).

The National Programme for the Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke (NPCDCS) was initiated in 100 districts in 2010, and expanded to about 468 districts in 2012.

During the 12th Plan, the activities under the programme include strengthening of health infrastructure by setting up of NCD clinics, providing necessary manpower for programme activities, health promotion activities, screening, early diagnosis, treatment and referral of patients suffering from these diseases through public health delivery system.

Recently, as a part of the Comprehensive Primary Health Care initiative the MoHFW, GoI, has issued guidelines to strengthen Sub- Centres as Health and Wellness Centres (H&WC), staffed by appropriately trained primary health care team. The Medical officer of the Primary Health Centre would oversee the
functioning of the SC/HWC that falls in that area. The risk assessment, screening, referral, and follow up for selected NCDs amongst all women and men aged 30 years and above, would be included in the set of services being offered as part of comprehensive primary health care.

To support the NCD division of MoHFW, GoI in successful roll out and expansion of interventions under the NPCDCS program and other NCD programs like deafness control, blindness control etc., under the overall umbrella of the Comprehensive Primary Health Care initiative, it is proposed to establish of a Technical Support Unit (TSU) for NCD in partnership with UNDP. The overall aim of the TSU will be to augment the capacity of the NCDs division of MoHFW and provide the requisite technical support and assistance. The proposed TSU will be led by JS (NCD) and Director (NCD) and will comprise a team of techno-managerial specialists.

A) Duties and Responsibilities:

The Technical Consultant will coordinate with the States/UTs and will be responsible for supporting in implementation of Rare Diseases in the State/UTs.

The Technical Consultant-Rare Diseases will support the Rare Diseases division in:

• Developing strategies for implementation of interventions under the Rare Diseases framework as per the National Rare Disease Policy 2021, issued by the MoHFW;
• Provide technical guidance to a team of technical experts/project staff of the TSU;
• Defining timelines for each activity pertaining to Rare Diseases in consultation with JS (Rare Diseases)/Addl DDG (Rare Diseases)/Director (Rare Diseases) and keep a track of these timelines;
• Facilitate support [such under Rashtriya Arogya Nidhi (RAN) etc] extended to the patients suffering from various rare diseases;
• Coordinate with the eight Centres of Excellence (CoE) identified under the policy, viz, All India Institute of Medical Sciences, New Delhi; Maulana Azad Medical College, New Delhi; Sanjay Gandhi Postgraduate Institute of Medical Sciences, Lucknow; Post Graduate Institute of Medical Education and Research, Chandigarh; Centre for DNA Fingerprinting & Diagnostics, Hyderabad; King Edward Medical Hospital, Mumbai; Institute of Postgraduate Medical Education and Research, Kolkata; and Center for Human Genetics with Indira Gandhi Hospital, Bengaluru;
• Sharing best practices and cross learning among the States and promoting adaptation of the innovation among the Districts;
• Place and offer need-based support to ensure execution of critical interventions in the Rare Diseases;
• Provide technical support for IEC activities for creating awareness amongst the general public, patients & their families and doctors;
• Capacity building of doctors for early and accurate diagnosis, standardization of diagnostic modalities and development of newer diagnostic and therapeutic tools;
• Builds capacity as per need to ensure regular and high-quality data in line with the monitoring and evaluation framework developed;
• Drafting Operational Guidelines and implementing the same with emphasis on appropriate preventive efforts directed at primary, secondary and tertiary levels of health care;
• Drafting documents such as for capacity building of the States and the healthcare institutes;
• Develop concept notes on relevant operations research topics related to the Rare Diseases;
• Develop analytical frameworks for demonstrating programmed impact. Identify critical issues in program implementation;
• Technical support, monitoring of the portal for crowdfunding for rare disease through coordination with all stakeholders;
• Liasioning with the support groups, patient-parent organisations, civil societies etc;
• Any other support solicited by JS (Rare Diseases)/Addl DDG (Rare Diseases)/Director (Rare Diseases);
• Undertakes travel as and when necessary to ensure effective implementation of activities and provide technical inputs to address gaps and bottle necks;
• Ensure regular and timely generation of reports on activities, outputs and outcomes to meet internal UNDP requirements and for the MoHFW;
• Support the development of periodic status reports, annual reports, case studies, etc. in a timely manner.

B) Competencies:

• Has a very good knowledge and experience in M&E and data management of national programs

C) Required Skills and Experience:

Education:

• MD in Community Medicine/MD in Community Health Administration/MDS in Public Health Dentistry
• MBBS/BDS with MPH
• Basic Sciences with relevant Public Health Experience

Experience:

• A minimum of 2 years of professional work in health programme and/or project development and management including programmatic management, research and monitoring at National or State level.
• Proficient in MS Office

Language:

• Fluency in English (spoken and written);
• Fluency in Hindi (spoken and written);

Age Limit:
• Upto 50 years
